The D0904 form is a referral document that allows GPs to direct Department of Veterans’ Affairs (DVA) clients to other health service providers, including psychologists at Veteran MindFocus.
3. Am I eligible for psychological services through the DVA?
Eligibility depends on your circumstances, including your service history and the type of DVA card you hold. Generally, veterans with Gold or White cards are eligible for psychological services.
4. What types of psychological services are available to veterans?
At Veteran MindFocus, we offer individual therapy, polyvagal therapy, group therapy, cognitive-behavioural therapy (CBT), trauma-focused therapy, and more. Our services provide support for PTSD, anxiety, depression, and other mental health concerns.
5. How many sessions can I have with a psychologist?
Typically, a referral allows for up to 12 sessions or one year of treatment, whichever comes first. After this period, you may need a new referral to renew them.
6. Can I access telehealth services if I cannot attend in person?
Yes, we provide telehealth services for veterans who cannot attend appointments in person. This allows you to receive the same level of care remotely.
7. What should I bring to my first appointment?

For non-urgent issues, schedule an appointment with your GP or a psychologist at Veteran MindFocus Psychology. If your need is urgent, please contact a crisis service or emergency support line immediately.
10. Are there any costs involved?
If you are eligible and have prior approval from the DVA, there are no costs for psychological services at Veteran MindFocus Psychology.
